How do I qualify for home health care?
Home health care is a resource dedicated to those who lack accessibility to the care they need or have special requirements for their care.
Our requirements to qualify for home health care services include:
- A physician’s order
- A skilled need; for example, nursing, physical therapy, etc.
- Being considered homebound, which means leaving home requires much forethought, planning, and/or assistance

- A physician certifies a terminal illness with a life expectancy of six months or less if the illness follows its expected course
