How to File a Complaint

We believe in transparency and providing clear pathways for addressing concerns. Should you need to file a complaint, please follow the guidelines below:

  1. To File an Employment Complaint:

If you wish to file a complaint related to employment, you must contact your Agency’s EEO (Equal Employment Opportunity) Counselor within 45 days from the date of the alleged discriminatory act, event, or personnel action.

  1. To File a Program Complaint (Civil Rights Discrimination):

To file a Civil Rights program complaint of discrimination, you can:

  • Complete the form online.
  • Obtain the form at any USDA office.
  • Call (866) 632-9992 to request the form.
  • Write a letter including all the information requested in the form.

Michigan Civil Rights Protections

In addition to federal protections, Schoolcraft Memorial Hospital adheres to the Elliott-Larsen Civil Rights Act (Act 453 of 1976), which defines civil rights and prohibits discriminatory practices, policies, and customs in Michigan. This act specifically protects against discrimination based on:
