If you wish to file a complaint related to employment, you must contact your Agency’s EEO (Equal Employment Opportunity) Counselor within 45 days from the date of the alleged discriminatory act, event, or personnel action.

To file a Civil Rights program complaint of discrimination, you can:

  • Complete the form online.
  • Obtain the form at any USDA office.
  • Call (866) 632-9992 to request the form.
  • Write a letter including all the information requested in the form.

Michigan Civil Rights Protections

In addition to federal protections, Schoolcraft Memorial Hospital adheres to the Elliott-Larsen Civil Rights Act (Act 453 of 1976), which defines civil rights and prohibits discriminatory practices, policies, and customs in Michigan. This act specifically protects against discrimination based on:

The act also preserves the confidentiality of records regarding arrest, detention, or other disposition without a conviction, and outlines the powers and duties of the civil rights commission and department, providing remedies and penalties.
