Medical Nutrition Therapy

At Schoolcraft Memorial Hospital, our Registered Dietitian provides evidence-based Medical Nutrition Therapy (MNT) to support patients across the continuum of care—from acute hospitalization to recovery and long-term disease management. MNT is a personalized, therapeutic approach to nutrition that uses comprehensive assessment, nutrition diagnosis, and individualized intervention to improve health outcomes and support healing.

Inpatient Nutrition Services

In the hospital setting, our dietitians work closely with physicians, nurses, and the interdisciplinary care team to address complex medical conditions such as diabetes, heart disease, kidney disease, gastrointestinal disorders, malnutrition, wound healing, critical illness, and post-surgical recovery. Nutrition plans are tailored to each patient’s medical condition, lab values, medications, cultural preferences, and ability to eat safely.

  4. Enteral and parenteral nutrition management
  5. Patient and caregiver education

Outpatient MNT services are available for individuals seeking ongoing nutrition support after discharge or for those managing chronic conditions in the community.

During outpatient visits, dietitians provide:

  • Goal setting and practical education to support disease management
  • Weight management
  • Blood sugar control
  • Heart health
  • Gastrointestinal conditions
  • Overall wellness.

Care plans are designed to be realistic, sustainable, and aligned with each patient’s lifestyle.
